GENERAL EXPLANATIONS

Data subjects described as the relevant person (hereinafter referred to as the “Applicant”) under the Law No. 6698 on Protection of Personal Data (“PDP Law”) are granted the right to make certain requests regarding the processing of their personal data pursuant to Article 11 of the PDP Law.

Pursuant to paragraph one of Article 13 of the PDP Law, applications regarding these rights must be submitted to our Company acting as the data controller, in writing or through other methods specified by the Personal Data Protection Board (“Board”).

Accordingly, written applications to our Company may be submitted by printing out this form and:

• by the Applicant in person,
• via notary public or
• by signing with a secure electronic signature as defined under the Electronic Signature Law No. 5070 and sending it to the registered electronic mail address of the Company,

and may thus be submitted to us.

Below, you will find details regarding the specific channels through which written applications should be submitted to us.

Further, once the other methods determined by the Board are announced, our Company will also announce how applications will be accepted through these methods.

Your application submitted to our Company will be responded to within thirty days from the date of receipt, depending on the nature of the application, in accordance with paragraph 2 of Article 13 of the PDP Law. Our response will be provided to you in writing or electronically in accordance with Article 13 of the PDP Law.

This application form has been prepared to determine your relationship with our Company, if any, identify in full the personal data processed by our Company and respond to your application properly and within the statutory period. Our Company reserves the right to request additional documents and information (copy of identity card or driving licence, etc.) for identification and authorization in order to eliminate legal risks that may arise from unlawful and unfair disclosure of data and, in particular, to ensure the security of your personal data. Our Company shall not accept any liability arising from inaccurate information or unauthorized applications in the event that the information you provide in this form regarding your requests is not accurate and up to date or an unauthorized application is made.
