GENERAL EXPLANATIONS

Data subjects described as the relevant person (hereinafter referred to as the “Data Subject”) under the Law No. 6698 on Protection of Personal Data (“Law”) are granted certain rights in connection with the processing of personal data pursuant to Article 11 of the Law.

Pursuant to paragraph 1 of Article 13 of the Law, applications regarding these rights must be submitted to our Company acting as the data controller, in writing or via other methods specified by the Personal Data Protection Board (“Board”). Accordingly, written applications to our Company should be submitted using this form:

by delivering it in person to the Quality & Business Processes Audit Department,
via notary public to [Değirmen Sok.Nidakule İş Merkezi No:18 K:5 Kozyatağı/İstanbul] and by signing it with a “secure electronic signature” as defined under the Electronic Signature Law No. 5070

and must be submitted to us accordingly.

Below, you will find details regarding the specific channels through which written applications should be submitted to us.

The channels listed above are “written” application channels in accordance with paragraph 1 of Article 13 of the Law. Once the other methods determined by the Board are announced, our Company will also announce how applications will be accepted through these methods.

Your application submitted to us will be answered “as soon as possible and at the latest within thirty days” from the date on which your request is received, depending on the nature of the request and in accordance with paragraph 2 of Article 13 of the Law. Our responses will be provided to you in writing or electronically in accordance with Article 13 of the Law.

The information and documents requested from you depending on the nature of this form and your request must be provided to us completely and accurately. If the requested information and documents are not properly provided, there may be difficulties in conducting the investigations to be carried out by our Company in relation to your request fully and properly. In such a case, our Company reserves its legal rights. Therefore, the relevant form must be submitted completely and together with the information and documents requested depending on the nature of your request.
