APPLICATION FORM

 

OVERVIEW

The Law No. 6698 on the Protection of Personal Data (the "Law on PPD") grants the personal data owners, who are referred to and defined as data subjects within the Law, (hereinafter to be referred to as the "Applicant") the right to file certain requests in respect of the processing of their personal data under Article 11 of the Law on PPD.

According to the first paragraph of Article 13 of the Law on PPD; the applications to be filed with the Company, being a data controller, in respect of such rights should be issued in writing or be filed with out party through other methods to be specified by the Board of Protection of Personal Data (the "Board").

Accordingly, through the course of the filing of a written application with the Company, this form should be completed and printed out, and

• be delivered in person by the Applicant,
• be sent through the agency of a notary public or
• Secure electronic signature created in accordance with the Electronic Signatures Law No. 5070 shall be affixed to the print-out of this form, and such copy should be sent via email to the registered electronic mail address of the Company.

Information as to how written applications are to be delivered to our party specifically through each method is provided as follows.

In addition; should the Board announce any other method of application, the designated form of receipt of applications through any such other method shall also be announced by the Company.

Your applications received by the Company will be replied within a period of thirty days as of the date of our receipt of your request, depending on the nature of the request, pursuant to the provisions of the second paragraph of Article 13 of the Law on PPD. Our replies will be sent to you in writing or electronically in accordance with the provisions of Article 13 of the law on PPD.

This application form has been issued in order for the determination of your relation with the Company, the establishment of any and all your personal data processed by the Company, if any, and the provision of your party with an appropriate reply to your relevant application accurately and within the legally prescribed period. The Company reserves the right to request additional documents and information (duplicate copy of ID card or driver's license etc.) for the verification of identity and establishment of authority with a view to eliminate any legal risks that may arise out of unlawful and wrongful disclosure of data and especially to ensure the security of your personal data. The Company hereby disclaims any liability in respect of any requests filed through applications that contain any information that is not accurate or up-to-date or that is unauthorized.
